List of documents required for embryos to be accepted for cryopreservation
For both of the Intended Parents:
- a marriage certificate
- passports
The exporting clinic needs to provide:
- The document of genetic relation to the embryo. One of the intended parents must be genetically related to the embryo.
- The viral-free status of the embryos. Both parents must have the serology test results. They need to be actual at the
moment of embryo creation. - All details of embryo creation and vitrification (including the name(s) of the medium(a) used to provide those procedures)
All of the documents must be provided in hard copies.
The shipping ( ship + cost) must be arranged and covered by the Intended Parents
Please note:
It is mandatory to have the following tests:
- The invasive test (to obtain biological material from the fetus for subsequent analysis of its DNA and to compare it with the parent’s DNA).
- The chorionic villus sampling – in the first trimester of pregnancy (10-14 weeks). The study of chorionic cells contains the same genetic material as the fetus. The risk of
miscarriage lowers to 1% after doing this test. Please pay attention! It will cost 1000 Euros additionally.
List of documents required for embryos to be accepted for oocyte cryopreservation
The exporting clinic needs to provide the following materials/documents:
- Passport
- Oocyte freezing protocol
- RW, HBs, HCV, HIV, syphilis
- TORCH infections (rubella and toxoplasmosis)
- Vaginal discharge analysis
- Blood group and Rhesus factor
- Complete Blood Count (CBC)
- Biochemical blood count (bilirubin, AST, ALT, creatinine, complete protein)
- Coagulation testing
- Blood-alcohol test
- Blood Drug Testing
- Nicotine testing
- LH (luteinizing hormone)
- FSH (follicle-stimulating hormone)
- Vaginal and cervical canal swab
- Karyotyping
- Cystic fibrosis testing
- Microscopic examination of the vaginal canal
- Chest X-ray
- Electrocardiogram (ECG)
- Pelvic organs ultrasound
- Breast ultrasound
- Psychological evaluation
- Phycological evaluation is done to see if the girl is psychologically healthy and ready for the donation
- OB/GYN opinion

List of documents required for embryos to be accepted for semen cryopreservation
The exporting clinic needs to provide the following information:
- Passport
- Sperm freezing protocol
- The results of the serology test (HIV, Hep B & C, and syphilis) which was made for male. It must be valid on the day of sperm collection – but not older than 3 months;
- The karyotype;
- The method used for sperm cryopreservation.
All of the documents must be provided in hard copies.
